How do I get services?

GATEWAY is open 24 hours a day, seven days a week. The phone number is: 1-800-973-GATE (4283). GATEWAY can set up an appointment for you. You can choose from many providers that GATEWAY contracts with.

Can I still see the provider that I have been seeing for years?

YES. You may choose from any of the providers that are GATEWAY's provider network.

What is Out-of-Network?

If you choose to get services not covered by GATEWAY or available within its network, you must get pre-approval from GATEWAY so that your care can be paid.

What happens if I have to be hospitalized?

You may call GATEWAY at 1-800-973-GATE (4283). GATEWAY customer service representative will help you get to a hospital that is contracted with GATEWAY. If the provider is outside the GATEWAY provider network, you must contact GATEWAY to get prior approval.

Do I have to stay in the GATEWAY program?

NO. If you want to choose another managed care provide network instead of GATEWAY, we will assist transfer to the new network. Persons requesting a change must be referred to the Agency's YOUR CHOICE ® coordinator at 1-888-490-9698 to initiate the transfer.

What is my Person-Centered Plan?


Person-Centered-Planning (PCP) is a way of helping you choose services and supports you need to achieve the goals you have set for your future. PCP allows you to be an active participant in your care.

How do I get an Independent Facilitator?

You may ask for an Independent Facilitator if you need help with your Person-Centered-Plan. GATEWAY will provide an Independent Facilitator at your request.
